Uploading and referencing images
How can I upload an image and reference it in the case log at the same time so that the image is rendered with the comments rather than as a link?

You can use Command ⌘ instead of Control ^ on Mac

You can't, without knowing the image ID beforehand. We'll have automatic previews sometime this month though. There's a big overhaul on attachments coming after our overhaul of the billing system.
