Lots of people have uploaded pics. There's an issue if you try to replace one with another using the same extension, and I seem to be getting intermittent Errno::ENOENT errors. Seems to be an odd case where it's pointing to a file path on an old deployment, but I haven't been able to track it down.
Hmm, just replaced my avatar with another using the same extension. Weird.

For further info: it happened on the first upload attempt. And on all subsequent attempts too.
Also: using Safari (if that can matter).

Worked for me in safari. I removed my avatar and re-uploaded it.

It is puzzling... I tried it with FireFox, and it's exactly the same behavior.

Now it finally worked. I had to remove the picture first, and then upload. But that was not at all intuitive, because there was no picture there to start with, and I never managed to upload any before. Anyway, now it's there!

Remove what picture? It shouldn't show that if you hadn't uploaded anything.

Exactly: What picture did it remove??? The remove link was well displayed there the very first time I went into the screen. The only picture displayed was the "anonymous" sillhouette gravatar (....lighthouseapp.com/images/avatar.gif), and it showed the 'remove' link. I clicked on remove. The anonymous gravatar picture was still displayed, but then I could finally upload a picture of my own choice.

Oh yea, forgot about that. At any rate, I deployed a large update to the avatar code. Should be good now.
