How can I keep track of time spent on an issue?
I've tried the sister application Tender too, but there too I was not able to locate how to report time, and then at the end of the month get a report of time spent

Sorry to ask another question - but how to I setup a "fully private" Tender setup? I do not want to mark each question as private - but want everything to be private - always

TIme tracking is not part of the workflow process for Tender or Lighthouse. It's an area we open sourced our tools that we use for time tracking, which we consider independent of actual workflow in those two apps.
http://github.com/entp/xtt is what we use via an AIM bot/hosted on our server.
In Tender, if you go to your Site Settings, there is a Private Mode options on the first page. Enable that, and it should be what you are looking for.
